Address Changes
Each
licensee must inform the board in writing of any changes
in physical and mailing addresses for their residence and
all locations where he or she conducts activities related
to the practice of chiropractic and/or acupuncture, including
removals from places of practice within thirty days of such
change.
If
a Post Office Box address is filed with the Board, the physical
location of the address must be disclosed to the Board.
Failure
to receive notification of renewal due to failing to advise
the board of a change of address does not constitute an
error on the part of the board, nor does it exonerate the
licensee from making such renewal.
All
changes of address must be filed with the Board in writing.
